Provide supporting information for Disability Living Allowance (DLA) for children
Use this service to respond to our request for supporting information.
We have already received some information from the child’s parent or guardian as part of their application. However, we need some more information about the child’s day to day needs to help us make a decision.
The information you provide will help us to understand the level of support that the child needs.
This information may also be seen by their parent or guardian upon request.
We will treat your information carefully. We may use it for any of our purposes. To learn about your information rights and how we use information, see our personal information charter.
Using the service
You will not be able to save and return to your answers. For your security, we will also delete your answers if you are inactive on the service for 1 hour.
Make sure you have all of the information you need before you start.
What information will I need?
You will also need any relevant information you have about the child, including:
- any individual plans in place
- any additional learning support that they get, or would benefit from
- their behaviour at school
- any support they need to move around at school
- how they manage their own personal safety
- how they manage their own personal care
- any therapy or medication while at school
- any sleep problems
